Medical Leave Guidelines
(Leave of Absence)

The request for Medical Leave must be presented to the Executive Board and requires a signed and dated request and a doctor’s letter, if appropriate. The fees will be as follows:
The first 90 days will be at 60% of the regular dues.
The second 90 days will be at 40% of the regular dues.
The third 90 days will be at 20% of the regular dues.
The fourth 90 days will be at zero percent of the regular dues.
It is important for a member requesting medical leave to do so in writing stating his/her request and reason. Your medical leave status will begin as soon as the office receives your letter, not months after you decided you cannot play. There will be no quarterly refunds. Percentages of quarterly dues will begin at the beginning of the following quarter, in which Cardinal Racquet Club receives a letter of request for medical leave.
The dues will be prorated if the member returns during a 90-day period.
If the member’s status is a couple or family and only one person is requesting a LOA, the discounts apply to the difference between the single rate and the couple or family rate.
A leave may be granted at the discretion of the Executive Board effective at the date of the request.
